Özet

Amaç: Obezite, genetik, çevresel ve davranışsal faktörlerin etkileşimi sonucu ortaya çıkan multifaktöriyel bir halk sağlığı sorunu olup, prevalansı dünya genelinde giderek artmaktadır. Son yıllarda obezitenin yalnızca yaşam tarzı faktörleri ile açıklanamayacağı, prenatal dönemde maruz kalınan çevresel ve beslenmeye bağlı etkilerin yaşamın ilerleyen dönemlerindeki metabolik sağlık üzerinde belirleyici rol oynayabileceği düşünülmektedir. Bu kapsamda fetal programlama kavramı, intrauterin dönemde meydana gelen çevresel maruziyetlerin bireyin ilerleyen yaşam dönemlerinde obezite ve metabolik hastalık gelişim riskini etkileyebileceğini öne sürmektedir. Bu derlemede, maternal beslenmenin fetal programlama ve obezite gelişimi üzerindeki etkilerinin epigenetik mekanizmalar çerçevesinde değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Bu makale kapsamda PubMed, Web of Science, Scopus ve Google Scholar veri tabanlarında fetal programlama, maternal beslenme, prenatal dönem, epigenetik mekanizmalar ve obezite ilişkisini ele alan çalışmalar incelenmiştir. Bulgular: Mevcut bulgular, maternal obezite, yüksek yağlı diyetler, enerji ve besin ögesi yetersizliklerinin fetal metabolik programlama üzerinde etkili olduğunu göstermektedir. Özellikle DNA metilasyonu, histon modifikasyonları ve RNA temelli düzenlemeler gibi epigenetik mekanizmaların enerji homeostazı, iştah düzenlenmesi, adipogenezis ve glukoz metabolizmasında görev alan genlerin ekspresyonunu etkileyerek yaşamın ilerleyen dönemlerinde obezite riskini artırabileceği bildirilmektedir. Ayrıca makro ve mikronütrient örüntüsünün fetal metabolik sağlık üzerinde belirleyici rol oynadığı, özellikle metil donörü besin ögelerinin epigenetik süreçlerde önemli işlevler üstlendiği görülmektedir. Sonuç: Sonuç olarak maternal beslenmenin prenatal dönemde metabolik programlama üzerinde kritik rol oynadığı ve çocukluk çağı ile erişkin dönem obezitesinin önlenmesinde gebelik öncesi ve gebelik süresince yeterli ve dengeli beslenmenin desteklenmesinin önemli bir halk sağlığı yaklaşımı olduğu düşünülmektedir.
